본 연구는 춘천시에 위치한 2개 종합병원 응급의료센터를 방문한 65세 이상 노인의 특성을 파악하고 한국형 응급환자 중증도 분류체계(KTAS)로 평가한 중증도 영향 요인을 파악하기 위한 것이다. 자료 수집은 2016년 1월, 2월, 12월에 응급의료센터를 방문한 65세 이상의 노인 2,496명의 전자의무기록을 통해 후향적으로 실시하였다. 수집된 자료는 SPSS/WIN 22.0 프로그램을 이용하여 카이제곱(chi-square test), 다중로지스틱 회귀분석(multiple logistic regression)으로 분석하였다. 응급의료센터 방문 노인 중 여성이 53.5%를 차지하였으며, 85세 이상 노인은 17.6%였다. 응급실 방문시 구급차를 이용한 대상자는 33.9%였다. 응급의료센터 방문 노인의 2/3가 응급에 속하였고 단변량 분석에서 중증도에 통계적으로 유의한 차이를 나타낸 요인은 어지러움, 호흡곤란, 흉통, 복통, 혈청 알부민수치였다. 이 중 중증도에 영향을 미치는 요인은 어지럼증(OR=22.01, p<.001), 호흡곤란(OR=5.32, p<.001), 흉통(OR=3.61, p<.001), 저알부민혈증(OR=1.41, p<.001)이었다. 노인의 불필요한 응급실 방문을 감소시키고 방문 노인에게 신속한 간호를 제공하기 위해중증도에 영향을 미치는 요인인 주호소와 저알부민혈증 관리를 위한 대책 마련이 요구된다.

Purpose: This study aimed to identify characteristics and severity assessed by Korean Triage and Acuity Scale (KTAS) among 2,496 older adults aged 65 and over who visited an emergency department (ED) located in Chuncheon city.
Methods: Data were collected via electronic medical records from two hospitals from January to December of 2016. Chi square test and logistic regression were applied using SPSS 22.0.
Results: 53.5% of the participants were women and 17.6% of visits were done by aged 85 and older. ED visits by ambulance were 33.9%. More than two-third of older adults’ ED visits were emergent status. Severity of the ED visits of older adults were rated by KTAS. Commonly reported chief complaints of the ED visits were dizziness, dyspnea, chest pain, abdominal pain, and hypoalbuminemia. Among them, dizziness, chest pain, dyspnea, and hypoalbuminemia were related factors for being-emergent condition.
Conclusion: Strategies for older adults’ chief complaints are needed in order to reduce unexpected ED visits.
